The Six Rights of Medication Administration
Fill in the blanks
Right ______________ - Ensure the medication is given/taken via the proper route; orally, sublingual (under the tongue), rectally (suppository), etc.
*
Right ______________ - The MAR will have each medication listed for the corresponding medication time. The support person is to cross reference this medication information with the medication listed on the medication pack(s).
*
Right ______________ - Immediately after a medication is administered, the support person signs the MAR on the correct date and time for the appropriate medication administered.
*
Right ______________ - Ensure that the MAR and the medication pack(s) you are dispensing medication from have the individual’s full name clearly visible. Reading the name out loud helps to reduce the chance of a medication error. A photo of the individual is in front of the MAR.
*
Right ______________ - Ensure that the time and date on the MAR and the time and date on the medication pack(s) reflect the proper time and date of the medication you are administering. For PRN medications, ensure that the indicated amount of time between doses has passed.
*
Right ______________ - Ensure that the information on the MAR and the information on the medication pack(s) are the same in regard to the dosage of each medication. Ensure that the right numbers of tablets are present for that dose.
